I represent that I am the legal owner of the pets described in the correlating agreement form *

I release BUSHels of Fun Pet Care Services (BOF), its staff, owners, representatives, and agents from any and all liability which I or my dog may suffer including but not limited to injury, sickness, damage or death resulting from participation in daycare, overnight boarding, or any of our other services. *

I represent that my dog is in good health, is current on all required vaccinations (DHLPP, Rabies Bordatella), is free of worms, fleas, ticks, and lice, and has not been ill with any known contagious viruses in the last 30 days. *

I understand that while my dog is fully vaccinated, that vaccines are not guaranteed and there is a small risk that my dog may contract a contagious disease or illness. I agree that should this occur I am responsible for my own pet’s care and medical attention. *

I understand that although all dogs are fully supervised that incidents or injuries may occur from playing with the other dogs, which include but not limited to bites, scrapes, scratches, and sprains. *

I understand that the ownership of BUSHels of Fun has done their due diligence in performing security and background checks of the staff members entering my home and have deemed said staff trustworthy to enter the premise under no supervision *

I represent that my dog is socialized and has not harmed or shown threatening behaviours towards any person or any other dog. I understand BUSHels of Fun reserves the right to remove my dog from the play area and place my dog in a separate holding area should my dog display any unwanted behaviours. I understand that in the event my dog displays aggressive signs, I will be responsible in ensuring my dog is picked up promptly. *

In the event of an emergency situation with my pet, I give permission to BOF to transport my pet to the nearest veterinary clinic in their personal vehicle for immediate medical care. *
