I represent that I am the legal owner of the pets described in the correlating agreement form *

I release BUSH-els of Fun Pet Care Services (BOF), its staff, owners, representatives, and agents from any and all liability which I or my dog may suffer including but not limited to injury, sickness, damage or death during any and all obedience classes my dog(s) attends. *

I acknowledged that I have informed Bushels of fun of any health or temperament concerns that may be relevant for the training classes. *

I acknowledge that my dog is able and healthy enough to perform any practices during any training courses through BUSHels of Fun and that the practices that the BOF Trainer implements with my dog is structured through strategy, science and implemented through positive reinforcement. *

I agree that I will not leave my dog unattended at any time of the 4 week obedience classes. If I am not able to attend any class, I agree that I will find an able and willing individual to be in my place of supervising and training my dog. *

I understand that although all dogs are fully supervised that incidents or injuries may occur, which include but not limited to bites, scrapes, scratches, and sprains. In the event that intervention is necessary at any time, I agree that a representative of BUSHels of Fun can intervene appropriately and as necessary. *

I understand that by enrolling my dog into the Training Classes, that does not guarantee that my dog will not display undesirable behaviours at home, especially if I do not continue to implement training with them on a daily basis. *
